Understanding Hemorrhoids and When Home Treatment May Work

Hemorrhoids are swollen blood vessels in or around the rectum and anus. They affect millions of people each year—studies suggest that roughly 50% of adults experience hemorrhoid symptoms by age 50. Many people feel embarrassed to discuss them, but hemorrhoids are a common medical condition that responds well to various treatment approaches.

Hemorrhoids come in two main types: internal and external. Internal hemorrhoids develop inside the rectum and typically don't cause pain, though they may bleed or cause itching. External hemorrhoids form under the skin around the anus and can be quite uncomfortable, especially when irritated. Some people have both types at the same time.

Home treatment works best for mild to moderate hemorrhoid symptoms. Research shows that about 50% of people with hemorrhoids find relief through non-medical approaches within one to two weeks. The key is understanding which symptoms respond to home care and when you should see a doctor instead.

Hemorrhoids often develop from increased pressure in the rectal area. Common causes include straining during bowel movements, sitting for long periods, chronic constipation or diarrhea, pregnancy, obesity, and a low-fiber diet. Understanding what triggered your hemorrhoids can help you choose the most effective treatment strategy.

Practical takeaway: Most hemorrhoids improve on their own within one to two weeks, especially with home treatment. Keep track of your symptoms—note when bleeding occurs, how long pain lasts, and what makes symptoms better or worse. This information helps you decide whether home treatment is working or if you need to see a healthcare provider.

Over-the-Counter Creams, Ointments, and Suppositories

Drugstore shelves offer many over-the-counter (OTC) products designed to treat hemorrhoid symptoms. These products fall into several categories, and understanding what each does helps you choose the right one for your situation.

Topical creams and ointments work by numbing the area, reducing inflammation, or both. Popular ingredients include hydrocortisone (a mild steroid that reduces swelling and itching), witch hazel (an astringent that may reduce irritation), and lidocaine (a numbing agent). Products containing these ingredients can provide temporary relief from pain and itching within minutes to hours. Most people can safely use these products for one to two weeks, though some doctors recommend not using steroid creams for longer than seven consecutive days without medical guidance.

Hemorrhoid suppositories are small, bullet-shaped medications inserted into the rectum. They're particularly helpful for internal hemorrhoids since they deliver medication directly to the affected area. Common suppository ingredients include hydrocortisone, cocoa butter, and various soothing oils. Suppositories typically work best when inserted after bowel movements or before bed.

Medicated wipes and pads offer an alternative to toilet paper, which can irritate already-sensitive skin. Products containing witch hazel or hydrocortisone can be gentler than dry wiping and may reduce itching and discomfort. Many people find these especially useful when away from home.

Medicated pads and wipes cost between $5 and $12 per package and last about one to two weeks with regular use. Creams typically cost $8 to $15 and last two to four weeks. Before using any OTC product, read the label carefully. Some products aren't recommended during pregnancy, and ingredients can interact with other medications.

Practical takeaway: OTC products work best for temporary symptom relief—they reduce pain, itching, and irritation but don't cure the underlying hemorrhoid. Choose products based on your main symptom: pain relief calls for lidocaine-based products, while itching responds better to witch hazel or hydrocortisone. If symptoms don't improve after one week of OTC treatment, consider consulting a healthcare provider.

Fiber, Hydration, and Dietary Changes

Diet plays a central role in hemorrhoid development and recovery. Constipation and straining are major hemorrhoid triggers, and both result partly from inadequate fiber and water intake. Research published in medical journals shows that increasing dietary fiber reduces hemorrhoid symptoms in about 60% to 70% of patients.

Fiber works by softening stool and increasing its bulk, which promotes regular bowel movements without straining. The recommended daily fiber intake is 25 grams for women and 38 grams for men, but most Americans consume only about 15 grams daily. Whole grains, legumes, vegetables, fruits, nuts, and seeds all provide dietary fiber. Examples include beans (7 grams of fiber per cooked cup), raspberries (8 grams per cup), whole wheat bread (3 grams per slice), and split peas (16 grams per cooked cup).

Increasing fiber gradually prevents bloating and gas. Jump from 15 to 38 grams daily, and you'll likely experience uncomfortable digestive symptoms. Instead, add about 5 grams of fiber daily each week until you reach your target. Pair fiber increases with water intake—fiber works by absorbing water in the digestive tract, so insufficient water can actually make constipation worse.

Hydration deserves equal attention. Most health organizations recommend drinking roughly 8 to 10 glasses of water daily, though individual needs vary based on activity level and climate. Even mild dehydration can lead to harder stools and constipation. Many people find that doubling their water intake when starting higher fiber diets prevents digestive discomfort while improving bowel regularity.

Other dietary considerations include limiting foods that may worsen symptoms. Spicy foods, caffeine, and alcohol can irritate the digestive tract and worsen hemorrhoid discomfort in some people. If these foods trigger your symptoms, reducing them may provide relief.

Practical takeaway: Start a food and symptom journal for one week. Write down what you eat, how much water you drink, and any hemorrhoid symptoms that day. Most people notice improvement within three to five days of increasing fiber and water. If dietary changes alone don't resolve symptoms within two weeks, other treatment options may be necessary.

Sitz Baths and Topical Soothing Methods

A sitz bath is one of the oldest and most effective home treatments for hemorrhoids. The practice involves sitting in warm water that covers the area around the anus and buttocks. Medical research consistently shows that sitz baths reduce pain, itching, and discomfort in about 50% to 80% of people with hemorrhoids.

Sitz baths work through several mechanisms. Warm water increases blood circulation to the area, promoting healing. The moisture soothes irritated skin. Soaking relaxes the anal sphincter muscles, which reduces pain. The process also gently cleans the area without harsh scrubbing.

A basic sitz bath requires only a bathtub or a special sitz bath basin (a small, inexpensive device that fits over a toilet seat). Fill the tub or basin with 3 to 4 inches of warm—not hot—water. Water temperature should be around 104 to 113 degrees Fahrenheit (40 to 45 degrees Celsius), roughly the temperature of a comfortable hot tub. Sit in the water for 10 to 15 minutes. Repeat this process two to three times daily, especially after bowel movements and before bed.

Many people enhance sitz baths by adding ingredients to the water. Unscented Epsom salt (about 1 to 2 cups) has been used traditionally and may reduce inflammation, though scientific evidence is limited. Witch hazel can be added for its astringent properties. Avoid bubble baths, soap, and heavily scented products, which can irritate the area.

For people without a bathtub, sitz bath basins cost between $15 and $30 and fit directly over toilet seats, making the treatment convenient and accessible. Some portable versions are available for travel.

Beyond sitz baths, other soothing methods include applying cool compresses to the area for 10 to 15 minutes several times daily, which numbs pain and reduces swelling. Some people use ice packs wrapped in soft cloth, while others find that cool witch hazel pads provide similar relief.

Practical takeaway: Sitz baths offer one of the most reliable
